Copy of Marks Card / Certificate
Attested copy of C- Form which the Trainee passed in the Trade Test
Attested copy of trainee’s admission list approved by the Joint Director (Training) of concerned Divisional office
Application Fee: Nill
Service Charge for Assisted Mode (Grama One/B1/K1/CSC): Rs 20/-
Service Time (Days): 10

Only for (G-c) citizen service not for Government departments and undertakings
